General Application Requirements

Bankruptcy:
MVLS application must be fully complete. Client must list all household members and their income information, completely answer all questions regarding their assets (including values), and sign and date MVLS Terms and Conditions Certification (page 5). Please send copies of:
- Tax returns from the IRS and State of Maryland for the previous three years.
- Client’s income information for the previous six months (e.g., six months of pay stubs, award letter for SSI/TCA/other benefits).
- Copies of two credit reports no older than six months. They must be from Experian (888-397-3742), Equifax (800-685-1111) or Transunion (877-322-8228). You can call 1-800-888-4213 or visit www.annualcreditreport.com to obtain free credit reports.
Family Law/Domestic
MVLS application must be fully complete. Client must list all household members and their income information, completely answer all questions regarding their assets (including values), and sign and date MVLS Terms and Conditions Certification (page 5). Please send copies of:
- One proof of income per source of income for all household members, such as: last paystub, benefits award letter for Social Security/TCA, etc., child support order/account history, etc.
- Court documents: any currently active orders and any recent complaints, motions/petitions, and answers filed since the case was last reopened.
- Adult Guardianship ONLY: you must also submit two (2) certificates completed by a physician, psychologist, or licensed/certified clinical social worker.
Non-Family Law
- Signed Certification of MVLS Terms & Conditions
- This can also be signed electronically by the client from a mobile smart phone with internet access. Clients can receive the Certification form by completing online intake or calling to complete telephone intake at 410-547-6537 or 800-510-0050.
- Any documents you have to support your case (contract agreements, receipts, written communication, etc.).
Tax
MVLS application must be fully complete. Client must list all household members and their income information, completely answer all questions regarding their assets (including values), and sign and date MVLS Terms and Conditions Certification (page 5). Please send copies of:
- PROOF OF INCOME for ALL members of your household. This can be:
- Last 3 paystubs for all household members who are employed.
- Award letter of statement of benefits from Social Security, SSDI, TDAP, SNAP, retirement income, veterans benefits, etc.
- Order for alimony or child support.
- Any documents showing any other income that you receive.
- TAX DOCUMENTS: Please send copies of any documents which describe your tax problem, such as:
- Notice or mail from the IRS or the Comptroller of Maryland
- Recent income tax returns
- IRS account transcripts
These documents need to show:
- The nature of the tax problem
- How much money is at issue, and
- The tax year(s) involved
